TRIAD PPO 2026 rates: how the EIN and age check works

TRIAD publishes a 2026 rate sheet. Here is how to read the age bands, answer the business EIN question, and understand the difference between a displayed rate and an approved price.

What the 2026 rate sheet actually shows

The published TRIAD sheet lists eight plan designs, four age bands (18–39, 40–49, 50–59 and 60–64), and four household types: member only, member plus spouse, member plus children, and family. Choose the age band and household that describe you before comparing monthly amounts.

For example, the sheet lists $628.05 per month for a member aged 18–39 on the 7350 Copay design. The same design lists $671.03 for a member aged 40–49. These are published pre-underwriting amounts, not personalized approvals or final premiums.

Why the page asks about your EIN

The TRIAD Individual PPO route is for a self-employed applicant with their own business EIN. The shopping page asks whether you have one before showing the TRIAD age-band estimate. If you are unsure whether your business meets the program rules, choose “Not sure” and ask an advisor to review eligibility.

Your name is optional and does not change the rate-sheet amount. Age determines the published band. Your household choice determines which row you see. The EIN answer is an eligibility screen, not a discount.

What happens after you see a number

TRIAD describes these prices as pre-underwritten average approved rates and states that medical underwriting is required. The final amount may be higher or lower, and approval is not guaranteed. State and program availability also need confirmation.

Compare the deductible, out-of-pocket limit, prescriptions and network terms for the exact design. The sheet is an initial benefits summary; its Schedule of Benefits and official plan documents govern coverage, limitations and exclusions. Cigna PPO identifies network access on the sheet, not the entity insuring the program.

Use the estimate as a shopping step

On the private PPO shopping page, enter your age, choose who is covered, and answer the EIN question. The TRIAD plan cards then show the matching published monthly estimate when the eligibility screen is met. Open a card to review its documents and ask for a case-specific eligibility review before applying.

Compare ACA Marketplace coverage and any available savings as a separate option, especially if your income has changed. Do not cancel existing coverage until a replacement's approval and effective date are confirmed.

Frequently asked questions

Are the rates on the page final quotes?

No. They match TRIAD's published 2026 pre-underwriting sheet for the selected age band, household and design. Medical underwriting can change the final rate, and approval is not guaranteed.

Do I need to enter my name to see a rate?

No. The name field is optional. Age, household and the EIN answer determine whether and which published TRIAD estimate appears.

What if I do not have an EIN?

The TRIAD Individual PPO route requires a self-employed applicant with their own EIN. You can still compare other private-plan options and ACA Marketplace coverage.

Does a Cigna PPO network name mean Cigna insures TRIAD?

No. The TRIAD sheet describes access to the Cigna PPO network and identifies the program as a self-insured plan reinsured by SiriusPoint. Review the governing documents for the entities and benefits of the exact option.
